NCOA Model Health Programs Toolkit: Healthy Changes

Healthy Changes is part of the Model Programs Project of the National Council on the Aging, funded by the Hartford Foundation. NCOA selected Portland (OR) as one of four cities to test model health promotion and wellness programs. The Portland Regional Advisory Panel developed the evidence-based model program, Healthy Changes, both an educational and support program designed to assist older adults in the day-to-day self-management of diabetes by focusing on diet and physical activity issues. The program offers weekly group meetings in community settings such as senior centers, community centers, congregate meal sites, and churches.
