Adopting Evidence-Based Depression Interventions for Older Adults
CDC's Prevention Research Centers-Healthy Aging Research Network (PRC-HAN) has created a set of 6 action briefs in conjunction with a webinar series on Healthy Aging and Depression. The action briefs address Overcoming Stigma, three recommended evidence-based programs, a Money Matters brief on funding, and The Basics on how organizations can get started in implementing evidence-based mental health programs.
State Coalition Resource Guide
NCOA, in collaboration with UNC, Chapel Hill and members of the State Coalitions on Falls Prevention Workgroup, has developed an online resource guide on building state coalitions for fall prevention. This guide includes tools, resources, strategies, and lessons learned from states that have developed their own coalitions. NCOA has compiled these resources into a framework of effective coalition building, liberally linking strategies to national web-based tools.
Using RE-AIM to Successfully Implement Evidence-Based Programs
Presented by Basia Belza, PhD, RN, of the University of Washington to the National Association of Chronic Disease Directors (NACDD) Healthy Aging Council, this presentation provided an overview of the goal, purpose and components of the RE-AIM framework and gave examples of how to use the framework for program planning and evaluation. A list of RE-AIM resources is also provided.
Review of Findings of CDSMP Outcomes
This report reviews the published literature on healthcare utilization and CDSMP. The evidence demonstrates that CDSMP results in significant improvements in patient outcomes and quality of life. The analysis also makes the "business case" for CDSMP, indicating that the program saves enough money through reductions in healthcare expenditures to pay for itself within the first year.
Financial Sustainability for Evidence-Based Programs
This document highlights some of the findings from a recent survey of people who are currently implementing, or have had experience working with, evidence-based healthy aging programs. In it, you will find some key ideas and tips for states and their partners to consider, many of which may help increase the likelihood of financial sustainability for evidence-based programs.
